Welcome
On behalf of our team of caregivers and support staff, thank you for considering our healthcare center. We offer a full range of medical services to meet the individual needs of our residents, including short-term rehabilitative services and long-term skilled care. Working with the resident’s physician, our staff – including medical specialists, nurses, nutritionists, dietitians and social workers – establishes a comprehensive treatment plan intended to restore you or your loved one to the highest practicable potential. Our goal is to provide quality care in a comfortable, compassionate environment, demonstrating on a daily basis our guiding principle of providing Quality Through People.

Overview
Conveniently located on 19th Avenue in San Francisco’s Sunset District, our healthcare center accommodates 140 residents and provides both short-term rehabilitative and long-term nursing care. Nineteenth Avenue is a 2004 recipient of the American Health Care Association’s Quality Award. Many of our caregivers are long-term employees who bring their warm and caring spirit to the daily care they provide. When a patient is admitted, our interdisciplinary team works with the resident and family to develop a comprehensive care plan that meets the resident’s individual needs. During the course of the resident’s stay, our staff strives to provide compassionate and supportive care that encourages the resident to achieve and maintain their highest practicable level of functional ability and independence. Services include 24-hour nursing care, social service, therapeutic activities and rehabilitative therapies as needed. Family and friends are encouraged to visit frequently. Parking is available and free to visitors.
